Data collection consent form

WHY DO WE COLLECT DATA?

ALL data that we collect in PRECURE, is with the purpose of understanding how to best reduce strain and help prevent injuries. Our goal is to help both companies and individuals to understand why strain occurs and take better care of our bodies while working.

The company receives the insights of the analyzed activities via a dashboard enabling data-based decision-making to address structural and organizational aspects.

Introducing big data and digitalization into the prevention of musculoskeletal disorders is a new approach within occupational health. In partnership with companies worldwide, we transform data into knowledge and turn knowledge into action. We set high standards, live by those, and strive to be a good example for others. We use data for the good of all and collaborate only with partners that share this ethic.

 

WHAT DATA DO WE COLLECT?

PRECURE’s devices collect physiological data such as muscle and motion data. The only motion data we collect, is your posture - the isolated movement of your body. This means that we are NOT able to see where you move or your location at any time, but only how you are positioned. The muscle data in combination with motion is used to analyze how you can best avoid strain and injuries.

To make individual assessments of certain motions and muscle loads, we compare the data to general individual attributes, like age, sex, height, weight. At no point will your personal attributes be shown unless in aggregated from in our interface – the individual attributes are only used in our own prediction of how much strain certain loads and motions cause.

To analyze the strain of different work activities, you will also be asked to log your work activities in the app. When you do these activities, and how much time you spent on them, will not be visible anywhere in our analysis – only the strain associated with the different activities across all participants are shown.

When you sign in with Microsoft, Google or Apple, the name and e-mail associated with your account is stored. The sole purpose and use of the e-mail is for you to be able to log you in again, linking you to your data – at no point will PRECURE contact or disclose this information to anyone.

For “Known” project type: The name you input will be visible in the MLI® Interface ONLY for your particular project, once you have submitted your project code. This means that help can be specifically targeted towards where you have the most strain.

For “Unknown” project type:. Your data will be associated with a pseudonym, and only the analysis of the whole group will be shown in the MLI® Interface. All data is pseudonymized and aggregated, meaning that your name won’t appear anywhere in the MLI® Interface The only place where your isolated personal feedback is visible, is in the MLI® app through your own login.

 

WHERE IS MY DATA STORED – AND FOR HOW LONG?


Securing your data is very important to us. We are very aware of the possible implications related to collecting data. To protect your personal information, we take reasonable precautions and follow industry best practices to make sure it is not inappropriately lost, misused, accessed, disclosed, altered, or destroyed.

We are complaint with GDPR, and other legislation to keep your data safe.

We assess the need for keeping your information and data after each project and at least annually. Weretain your personal data for as long as necessary to fulfill the purpose for which it was collected or to comply with legal, regulatory or internal policy requirements.

Data that is not part of any active usease or projects are irrevocably anonymized, meaning that all ties to individually identifiable information are removed. Data not necessary is deleted.

 

Data controller

WHAT ARE MY RIGHTS?

To the extent that the General Data Protection Regulation of the EU (GDPR) applies to our processing of your personal data (or other laws that include similar rights), you have several rights, of which we would like to inform you; the right to access your data, the right of data rectification (if your data is inaccurate), the right of erasure (if the retention of your data is no longer necessary in relation to the envisaged purpose of the processing), the right to restrict the data processing (e.g. if you contest the accuracy of your data that we process), the right to data portability and the right to lodge a complaint with the competent supervisory authority. At any time during which your data is processed by us, you can withdraw your consent to the processing by reaching out to PRECURE and request the data deletion (see how to remove your data below).

How to remove my data

You may at any point of time ask for your data to be deleted. In order to delete your data, you just need to provide the device number that was provided to you by means of your employer, therefore, your employer will communicate us the device number from which the data has to be deleted. In this way, the employer does not know which data belongs to you and we do not know who you are, so your identity remains anonymous.

How to withdraw my consent

Your participation in the pilot project is voluntary and you can withdraw your consent at any time. Your consent can be revoked by sending an e-mail to mail@precure.dk. The revocation of your consent does not affect the lawfulness of the processing that took place before the revocation. A revocation of the consent will mean that PRECURE will not be able to obtain, exchange or pass on further information about you after the revocation.

Use

PRECURE products may cause skin irritation.

Prolonged contact may contribute to skin irritation for some users. To reduce irritation, follow four simple wear and care tips:

·       keep it clean

·       don’t wear it too tight

·       give your skin a rest by removing the product for at least an hour after extended wear. It is not recommended to wear the product for more than 8 consecutive hours.

If any skin irritation is noticed, remove your PRECURE product immediately. If symptoms persist after removing the device, consult your medical professional.

PRECURE products contain electrical equipment that could cause injury if not handled properly. Proper handling includes, but is not limited to the following:

·       If you feel soreness, tingling, numbness, burning or stiffness on your skin while or after wearing the product, please immediately discontinue use.

·       Do not attempt to replace the battery, open the enclosure or disassemble your PRECURE product. Doing so will void the warranty and can result in a safety hazard.

·       Do not use your PRECURE product, if you can see it has been physically damaged.

·       Substances in a PRECURE product and its battery may harm the environment or cause injury if handled and disposed of improperly.

·       Do not expose your PRECURE product to extremely high or low temperatures.

·       Do not use your PRECURE product in or around water, in a sauna or steam room.

·       Do not use abrasive cleaners to clean your PRECURE product.

·       Remove your PRECURE product immediately if it feels hot.

·       Charge the battery in accordance with the instructions provided. Only use a power supply that is certified by a recognized testing laboratory and an authorized PRECURE charging cable.

·       Do not charge or use your PRECURE product if wet.

·       Consult your medical professional before use if you have any preexisting conditions that might be affected by your use of any PRECURE products or services.

·       Do not check the app or the product while driving or in other situations where distractions could be hazardous. Always be aware of your surroundings when using PRECURE’s products and services.

·       A PRECURE product is not a toy. Some PRECURE products contains small components that could be a choking hazard and are not intended for use by small children. PRECURE products are not intended for use by pets.

·       PRECURE products and services are not medical devices, and are not intended to diagnose, treat, cure, or prevent any disease. Regarding accuracy, PRECURE has developed products and services to track certain wellness information as accurately as reasonably possible.
